Tarlatamab + temozolomide is an investigational combination therapy being evaluated for the treatment of DLL3-positive high-grade central nervous system (CNS) tumors, including high-grade gliomas. Tarlatamab is a bispecific T-cell engager (BiTE) designed to bind to Delta-like ligand 3 (DLL3) on tumor cells and CD3 on T-cells, facilitating T-cell-mediated tumor cell lysis. Temozolomide is an oral alkylating agent that induces DNA damage by methylating guanine residues. This combination, studied in the TARLATEM trial sponsored by Centre Leon Berard in collaboration with Amgen, aims to leverage the immunotherapeutic activity of tarlatamab alongside the cytotoxic effects of metronomic temozolomide in patients with high-grade brain tumors.
