Technetium-99m + blue dye refers to the conventional dual-modality sentinel lymph node (SLN) localization technique used primarily in the surgical management of early-stage breast cancer and melanoma. The procedure involves the peritumoral or periareolar injection of a radioactive tracer, typically **technetium-99m (Tc-99m)** labeled to a nanocolloid (such as albumin or sulfur colloid), and a visual tracer, such as **isosulfan blue**, **patent blue V**, or **methylene blue**. The Tc-99m component allows for preoperative lymphoscintigraphy and intraoperative localization using a handheld gamma detection probe, while the blue dye provides direct visual identification of the lymphatic channels and the sentinel node. This combined approach is considered the standard of care, as it significantly improves the SLN identification rate and reduces the false-negative rate compared to using either tracer alone.
