Technetium Tc 99m-p5+14 is a radiopharmaceutical imaging agent consisting of the synthetic peptide p5+14 labeled with technetium-99m. The peptide is pan-amyloid reactive and binds specifically to amyloid deposits in tissues. When radiolabeled with technetium-99m, it enables visualization of amyloid-laden organs using SPECT/CT imaging. This agent has been developed for the detection of systemic amyloidosis—including AL (light chain) and ATTR (transthyretin) types—with particular focus on cardiac involvement (cardiac amyloidosis). Preclinical studies demonstrated high specificity and stability for detecting AA amyloid in animal models, while early clinical trials have shown promising results in human subjects for non-invasive diagnosis of cardiac amyloidosis. The mechanism involves binding to heparan sulfate proteoglycans present within the extracellular matrix of amyloid deposits.
