Teclistamab + daratumumab + lenalidomide is a triple combination regimen under investigation and in clinical development for treating multiple myeloma, including newly diagnosed patients ineligible for stem cell transplant and those with relapsed or refractory disease. Teclistamab is a bispecific monoclonal antibody that binds B-cell maturation antigen (BCMA) on myeloma cells and CD3 on T cells to redirect T-cell cytotoxicity toward malignant plasma cells. Daratumumab is an anti-CD38 monoclonal antibody that causes myeloma cell death via antibody-dependent cellular cytotoxicity, complement-dependent cytotoxicity, and apoptosis. Lenalidomide is an immunomodulatory small molecule that enhances immune response against myeloma cells and has anti-angiogenic and anti-proliferative properties. The combination aims to potentiate immune-mediated antimyeloma effects and deepen clinical responses. This triple regimen is being evaluated in multiple phase 1b and phase 3 clinical trials for efficacy and safety in various multiple myeloma settings[1][2][3][4][7].
