Teclistamab + lenalidomide is a combination of a **bispecific monoclonal antibody** (teclistamab) targeting B-cell maturation antigen (BCMA) and CD3 on T cells, and an **immunomodulatory small molecule** (lenalidomide). Teclistamab, marketed as TECVAYLI, binds BCMA on multiple myeloma cells and CD3 on T cells, redirecting T-cell cytotoxicity to induce lysis of malignant cells. Lenalidomide (Revlimid) is an immunomodulatory drug (IMiD) that promotes tumor cell apoptosis and modulates the immune response, enhancing T-cell and natural killer (NK) cell activity. The combination aims to produce synergistic effects, improving efficacy for patients with multiple myeloma, including those newly diagnosed or relapsed/refractory. Preliminary and phase 3 clinical trials have shown high rates of minimal residual disease (MRD) negativity, suggesting promising disease control. Adverse effects include neutropenia, lymphopenia, infections, mild to moderate cytokine release syndrome, and others typical of each individual drug[1][2][4][5][7].
