**Teclistamab + nirogacestat** is an experimental combination regimen for the treatment of relapsed or refractory multiple myeloma. Teclistamab is a bispecific antibody targeting B-cell maturation antigen (BCMA) and CD3, inducing T-cell–mediated cytotoxicity in BCMA-expressing plasma cells. Nirogacestat is a small molecule gamma-secretase inhibitor that increases membrane-bound BCMA and reduces its soluble form, potentially enhancing the efficacy of BCMA-targeting therapies. Clinical studies (notably MajesTEC-2) have shown high overall and complete response rates in heavily pretreated multiple myeloma patients, although safety concerns such as dose-limiting toxicities have warranted careful evaluation of dosing regimens[1][3][5][6][8].
