teclistamab + pomalidomide

Teclistamab + pomalidomide is an investigational combination therapy for the treatment of multiple myeloma, specifically being evaluated in the early relapse setting. Teclistamab is a humanized bispecific monoclonal antibody that functions as a T-cell engager, simultaneously binding to the B-cell maturation antigen (BCMA) on the surface of multiple myeloma cells and the CD3 receptor on the surface of T cells. This dual binding facilitates the recruitment and activation of cytotoxic T cells to eliminate BCMA-expressing malignant cells. Pomalidomide is a small molecule immunomodulatory drug (IMiD) that binds to the cereblon E3 ubiquitin ligase complex, leading to the degradation of transcription factors Ikaros (IKZF1) and Aiolos (IKZF3), which results in direct antineoplastic activity and enhancement of T-cell and natural killer cell-mediated immune responses. The National Cancer Institute (NCI) is sponsoring a Phase Ib/II trial (NCT05083169) comparing this combination against the standard triplet of carfilzomib, pomalidomide, and dexamethasone.
