Overview

This combination therapy consists of isolated cadaveric islet cells transplanted into patients with brittle type 1 diabetes mellitus, supported by tegoprubart (AT-1501) as an immunosuppressive agent. Tegoprubart is a humanized monoclonal antibody developed by Eledon Pharmaceuticals that selectively targets and inhibits the CD40 ligand (CD40L, also known as CD154), a key co-stimulatory molecule involved in T cell activation and the immune response against allogeneic transplants. By blocking the CD40/CD40L pathway, tegoprubart aims to prevent the rejection of the transplanted insulin-producing beta cells without the need for calcineurin inhibitors, which are often associated with significant side effects and toxicity to the islet cells themselves. The islet cells are typically infused into the portal vein of the liver, where they engraft and function to regulate blood glucose levels by producing insulin.
