Telbivudine + lamivudine is a combination of two nucleoside reverse transcriptase inhibitors (NRTIs), both of which are small molecule antivirals. Telbivudine is primarily used to treat chronic hepatitis B infection, where it inhibits viral replication by being phosphorylated to its active triphosphate form and causing DNA chain termination during the synthesis of the second strand of viral DNA[3][6]. Lamivudine is used in the treatment of both HIV and hepatitis B infections; it acts as a synthetic nucleoside analogue that, once phosphorylated intracellularly, inhibits reverse transcriptase and HBV polymerase by causing premature DNA chain termination[1][4][5]. Both drugs disrupt viral DNA synthesis but act at different stages—telbivudine mainly on DNA-to-DNA transcription and lamivudine on RNA-to-DNA reverse transcription. The combination may be considered for enhanced antiviral activity or resistance management in hepatitis B therapy.
