telcagepant + ibuprofen is a combination of two orally administered small molecule drugs: telcagepant, a calcitonin gene-related peptide (CGRP) receptor antagonist, and ibuprofen, a nonsteroidal anti-inflammatory drug (NSAID). This combination was investigated for the acute treatment of migraine headaches. Telcagepant blocks the action of CGRP, a neuropeptide involved in migraine pathophysiology and vasodilation, by competitively antagonizing the CGRP receptor. Ibuprofen exerts its effect by reversibly inhibiting the cyclooxygenase (COX-1 and COX-2) enzymes, decreasing prostaglandin synthesis and thus inflammation and pain. Co-administration was hypothesized to provide additive or synergistic effects due to complementary mechanisms. Clinical trials found numerically higher but not statistically significant improvement in pain freedom at 2 hours versus telcagepant alone, with a similar adverse event profile including fatigue, nausea, dizziness, somnolence, dry mouth, and tremor[1][3][6][7]. Telcagepant's development has since been discontinued due to concerns over hepatic safety during chronic dosing.
