A combination of **telmisartan**, an angiotensin II receptor blocker (ARB), and **dipyridamole**, a phosphodiesterase inhibitor and nucleoside transport inhibitor with antiplatelet and vasodilatory properties. Telmisartan selectively blocks the angiotensin II type 1 receptor, reducing vasoconstriction and aldosterone secretion, and may also act as a partial agonist at peroxisome proliferator-activated receptor gamma (PPARγ), influencing glucose and lipid metabolism. Dipyridamole inhibits platelet cAMP phosphodiesterase and cellular reuptake of adenosine, increasing cAMP and adenosine levels, thereby inhibiting platelet aggregation and causing vasodilation. This specific combination is not approved as a fixed-dose formulation, but both agents have been studied in parallel (often in a 2 × 2 factorial design) for secondary prevention of stroke, though not as a combined single product[1][2][3][4][5][6][7].
