Telmisartan + levoamlodipine is a fixed-dose combination (FDC) antihypertensive therapy developed by Jiangxi Shimei Pharmaceutical. It combines telmisartan, a non-peptide angiotensin II receptor blocker (ARB), with levoamlodipine, the pharmacologically active (S)-enantiomer of the calcium channel blocker amlodipine. Telmisartan selectively antagonizes the angiotensin II type 1 (AT1) receptor, thereby inhibiting the vasoconstrictive and aldosterone-secreting effects of angiotensin II. Levoamlodipine inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle cells, resulting in peripheral vasodilation and a reduction in systemic blood pressure. This combination provides a dual mechanism of action that targets both the renin-angiotensin-aldosterone system and calcium-mediated vascular contraction, offering enhanced efficacy in the management of essential hypertension. The use of levoamlodipine instead of racemic amlodipine is intended to maintain therapeutic potency while potentially reducing the incidence of dose-dependent side effects such as peripheral edema.
