Telmisartan + omega-3 fatty acids is a fixed-dose combination of two agents with complementary cardiovascular and metabolic effects. Telmisartan is an orally active, nonpeptide angiotensin II receptor blocker (ARB) that selectively antagonizes the type 1 angiotensin II receptor in vascular smooth muscle and the adrenal gland, leading to vasodilation, reduced aldosterone secretion, decreased systemic vascular resistance, and blood pressure lowering. It also acts as a partial agonist of peroxisome proliferator-activated receptor gamma (PPARγ), which may improve carbohydrate and lipid metabolism as well as insulin sensitivity[3]. Omega-3 fatty acids (primarily EPA and DHA), often derived from fish oil, are essential polyunsaturated fats that lower serum triglyceride levels by modulating hepatic lipogenesis pathways, increasing beta oxidation of fatty acids, reducing inflammation through effects on nuclear transcription factors such as NF-kB, improving endothelial function via nitric oxide synthase activation (eNOS), and exerting antioxidant properties[2][5][6][8]. The combination has been studied preclinically for synergistic benefits in conditions such as vascular endothelial dysfunction (VED) induced by oxidative stress or metabolic syndrome models like polycystic ovary syndrome (PCOS). In these settings it demonstrated additive improvements in glycemic control, antioxidant status, anti-inflammatory activity, lipid profile normalization, endothelial function restoration via eNOS activation/ATP-sensitive K+ channel opening[2][5], suggesting potential utility for cardiometabolic risk reduction.
