Temozolomide + vandetanib is an investigational combination therapy for glioblastoma and other malignant gliomas. Temozolomide is an oral alkylating agent that penetrates the blood-brain barrier and induces cytotoxicity by DNA alkylation. Vandetanib (also known as ZD6474) is a small molecule inhibitor targeting both vascular endothelial growth factor receptor 2 (VEGFR2) and epidermal growth factor receptor (EGFR), providing anti-angiogenic and anti-proliferative effects. The combination aims to enhance tumor control by simultaneously inducing DNA damage in tumor cells (temozolomide) and inhibiting angiogenesis as well as EGFR-mediated signaling pathways that contribute to tumor growth and resistance (vandetanib). Preclinical studies have shown significant reduction in glioblastoma tumor volume with this combination compared to monotherapy[1][6][8]. Clinical trials have evaluated its safety profile when combined with radiotherapy in newly diagnosed glioblastoma patients[2][3][4].
