Temozolomide + zotiraciclib is a combination therapy being investigated for the treatment of recurrent high-grade gliomas. Temozolomide is an alkylating agent commonly used to treat brain tumors, while zotiraciclib is a multi-kinase inhibitor that acts primarily as a CDK9 inhibitor. This combination works by depleting short-lived survival proteins such as c-MYC and MCL-1 which are overexpressed in glioblastoma, while also causing DNA damage through temozolomide's alkylating properties. Zotiraciclib penetrates the blood-brain barrier, making it particularly suitable for brain tumor treatment. The combination has shown promising results in clinical trials, particularly in patients with IDH1-mutated gliomas.
