Tenapanor + psyllium is a combination of two agents: tenapanor, a small-molecule inhibitor of the sodium/hydrogen exchanger isoform 3 (NHE3), and psyllium, a bulk-forming dietary fiber. Tenapanor acts locally in the gut by inhibiting intestinal NHE3, leading to reduced sodium and phosphate absorption, increased colonic fluid retention, and softer stool, resulting in increased stool frequency and weight. It is approved for the treatment of irritable bowel syndrome with constipation (IBS-C) and for reducing serum phosphorus in adults with chronic kidney disease (CKD) on dialysis. Psyllium increases stool bulk and water content, promoting regular bowel movements and is typically used as an over-the-counter laxative for constipation and to help regulate bowel function. The combination is not a uniquely approved or marketed drug but may be studied together to enhance therapeutic effect in constipation-predominant disorders, leveraging both mechanisms for additive or synergistic efficacy[1][2][4].
