Overview

Tenecteplase + enoxaparin is a combination therapy used primarily in the management of acute ST-elevation myocardial infarction (STEMI). Tenecteplase is a genetically engineered variant of tissue plasminogen activator (tPA) that acts as a thrombolytic agent, promoting fibrinolysis by converting plasminogen to plasmin, which then degrades fibrin clots. Enoxaparin is a low-molecular-weight heparin (LMWH) that acts as an anticoagulant by potentiating antithrombin III and inhibiting factor Xa and, to a lesser extent, factor IIa (thrombin), thereby reducing new clot formation. The combination aims to enhance reperfusion and reduce early ischemic events in patients with acute myocardial infarction. Clinical studies have shown this regimen can reduce reinfarction rates but may increase the risk of bleeding complications, particularly intracranial hemorrhage in elderly patients[1][2][4][7].
