Overview

This is a combination therapy used primarily in the management of acute ST-elevation myocardial infarction (STEMI). Tenecteplase is a genetically engineered variant of alteplase, functioning as a fibrin-specific plasminogen activator that promotes thrombolysis by converting plasminogen to plasmin, leading to clot dissolution. Unfractionated heparin is an anticoagulant that potentiates the activity of antithrombin III, inhibiting thrombin and factor Xa to prevent further clot formation. Clopidogrel is an antiplatelet agent that irreversibly inhibits the P2Y12 subtype of ADP receptor on platelets, reducing platelet aggregation. This combination aims to achieve rapid reperfusion by dissolving existing clots (tenecteplase), preventing new clot formation (unfractionated heparin), and inhibiting platelet aggregation (clopidogrel). The regimen has been shown to reduce ischemic complications and mortality in STEMI patients[1][2][3][4][5].
