This combination therapy represents an approach to managing the infection, particularly in cases where resistance to single-agent therapy has developed. **Mechanism of Action:** - **Tenofovir** is a nucleotide analog reverse transcriptase inhibitor (NRTI) that inhibits HBV DNA polymerase by competing with natural substrates, causing DNA chain termination and inhibiting viral replication. - **Telbivudine** is a nucleoside analog that inhibits HBV DNA polymerase by competing with thymidine, the natural substrate, leading to chain termination of DNA synthesis and inhibition of viral replication. Both medications are primarily eliminated through the kidneys and require careful monitoring, especially in patients with renal impairment. Tenofovir has a half-life of approximately 32 hours, while telbivudine's clearance is linearly proportional to renal function.
