This combination antiretroviral therapy (cART) regimen consists of the non-nucleoside reverse transcriptase inhibitor (NNRTI) rilpivirine, the nucleoside reverse transcriptase inhibitor (NRTI) emtricitabine, and a nucleotide reverse transcriptase inhibitor (NtRTI), which can be either tenofovir disoproxil fumarate (TDF) or tenofovir alafenamide (TAF). Marketed under the brand names Complera (with TDF) and Odefsey (with TAF), these fixed-dose combinations are standard treatments for HIV-1 infection. Beyond its antiviral properties, this specific regimen is being investigated in the Phase 4 MAFALDA-R clinical trial, sponsored by Fundacion SEIMC-GESIDA, for its potential to slow liver fibrosis progression in HIV-infected patients with metabolic dysfunction-associated steatotic liver disease (MASLD). Rilpivirine has demonstrated preclinical evidence of hepatoprotective effects, potentially attenuating liver inflammation and fibrosis by modulating hepatic stellate cell activation.
