Teriparatide + alendronate is a combination therapy used in the management of osteoporosis, particularly in patients at high risk for fracture or with prior bisphosphonate use. Teriparatide is a recombinant form of parathyroid hormone (PTH 1-34) that acts as an anabolic agent by stimulating osteoblast activity, increasing bone formation, and improving bone microarchitecture and strength. Alendronate is a bisphosphonate that inhibits osteoclast-mediated bone resorption, thereby reducing bone turnover and loss. The rationale for combining these agents is to leverage the anabolic effects of teriparatide while simultaneously suppressing excessive bone resorption with alendronate. However, evidence suggests that concurrent administration may blunt some of the anabolic benefits of teriparatide compared to monotherapy; sequential or cyclical regimens may be more effective in certain patient populations[1][3][5]. This combination has been studied primarily for osteoporosis treatment.
