Terlipressin + albumin is a combination therapy used primarily for the treatment of hepatorenal syndrome (HRS), particularly HRS-AKI (acute kidney injury) in patients with advanced liver disease. Terlipressin is a synthetic vasopressin analogue that acts as a vasoconstrictor, targeting splanchnic circulation to improve renal perfusion. Albumin, a plasma expander and colloid solution, is administered concomitantly to support intravascular volume and enhance the efficacy of terlipressin. This combination has demonstrated improved rates of HRS reversal compared to placebo or other therapies, but it carries an increased risk of adverse events such as respiratory failure, especially in vulnerable populations[1][2][3][4]. The mechanism involves both vasoconstriction via V1 receptors (terlipressin) and plasma volume expansion (albumin).
