Overview

**Tertomotide + gemcitabine + capecitabine** is a multi-drug combination regimen, primarily investigated as a treatment for cancers such as pancreatic cancer. Tertomotide (also known as KOC1 peptide vaccine) is a cancer vaccine designed to stimulate a patient’s immune system to recognize and attack cancer cells by targeting the KOC1 antigen, which is overexpressed in certain tumors. Gemcitabine is a nucleoside analog used as chemotherapy, which inhibits DNA synthesis by being incorporated into the DNA of dividing cells, leading to cell death; it is commonly used for pancreatic, lung, breast, and other cancers. Capecitabine is an oral prodrug that is metabolized to 5-fluorouracil (5-FU) inside the body, mainly in tumor cells; 5-FU acts as an antimetabolite, blocking DNA and RNA synthesis and promoting cell death[1][2][3][4][5]. This combination was investigated in clinical trials, primarily as neoadjuvant or adjuvant therapy following surgical resection in pancreatic cancer to improve immune response and reduce recurrence, but its development was terminated after phase II due to lack of efficacy. The mechanism includes direct cytotoxicity from gemcitabine and capecitabine, and immune activation against cancer cells by tertomotide.
