This multimodal therapeutic regimen was developed by the University Hospital, Clermont-Ferrand, to treat protein-energy wasting (PEW) in patients undergoing maintenance hemodialysis. The approach integrates pharmacological and non-pharmacological interventions to address the complex pathophysiology of undernutrition in chronic kidney disease. It consists of oral omega-3 fatty acid supplementation (n3 polyunsaturated fatty acids) to mitigate systemic inflammation, testosterone (an androgen) to promote protein anabolism and increase muscle mass, oral nutritional supplementation to provide essential calories and protein, and intradialytic exercise training (using an ergonomic bicycle) to improve physical function and effort tolerance. This synergistic strategy aims to improve the nutritional status and overall quality of life for undernourished hemodialysis patients.
