Overview
This is an investigational cannabis extract containing a 1:1 ratio of tetrahydrocannabinol (THC) and cannabidiol (CBD), developed by the Hadassah Medical Organization for use as an anesthetic premedication. The extract, manufactured by the Bazelet Group, is being evaluated in the Phase 2/3 'Cannapremed' clinical trial for its efficacy in managing postoperative pain, postoperative nausea and vomiting (PONV), and perioperative anxiety in patients undergoing elective surgery. THC acts primarily as a partial agonist at the CB1 and CB2 cannabinoid receptors, providing analgesic and antiemetic effects. CBD is included to potentially enhance therapeutic benefits while attenuating THC-induced side effects such as anxiety and tachycardia, likely through its activity at 5-HT1A receptors and other non-cannabinoid targets. The drug is administered sublingually as an oil-based solution shortly before anesthetic induction.
