TF2 + 177Lu-IMP-288 is a pretargeted radioimmunotherapy (PRAIT) system developed for the treatment of carcinoembryonic antigen (CEA)-expressing malignancies, including small cell lung cancer (SCLC) and non-small cell lung cancer (NSCLC). The system utilizes a two-step approach to deliver targeted radiation while minimizing systemic toxicity. The first component, TF2, is a humanized trivalent bispecific monoclonal antibody (bsMAb) created using 'Dock-and-Lock' technology; it features two binding sites for CEA and one for the hapten histamine-succinyl-glycine (HSG). After TF2 has localized to the tumor and cleared from the circulation, the second component, 177Lu-IMP-288, is administered. IMP-288 is a small peptide containing two HSG moieties and a DOTA chelator bound to the therapeutic radionuclide lutetium-177. The peptide rapidly binds to the tumor-localized TF2, delivering a concentrated dose of beta-radiation directly to the cancer cells. This pretargeting strategy aims to achieve higher tumor-to-background radiation ratios compared to directly radiolabeled antibodies, thereby reducing bone marrow exposure.
