A two-step pretargeted radioimmunotherapy combining the bispecific antibody TF2, which binds carcinoembryonic antigen (CEACAM5) and the histamine-succinyl-glycine (HSG) hapten, with the radiolabeled hapten-peptide 90Y-IMP-288 (DOTA-di-HSG). After TF2 targets CEACAM5-expressing tumors and clears from blood, 90Y-IMP-288 is administered and rapidly distributes; its bivalent HSG motif binds TF2 on tumor cells, delivering beta radiation from yttrium-90 to achieve selective tumor irradiation while minimizing normal tissue exposure. Developed clinically in colorectal cancer with imaging/therapeutic sequences using 111In/177Lu analogs and 90Y for therapy.[1][3][7]
