**Thalidomide + prednisolone** is a combination of two pharmaceuticals: thalidomide, an immunomodulatory agent with anti-inflammatory and anti-angiogenic properties, and prednisolone, a synthetic glucocorticoid corticosteroid with potent anti-inflammatory and immunosuppressive actions. The combination is primarily used off-label in hematological disorders such as primary myelofibrosis (PMF) and anemia, where standard therapies may not be effective or tolerated. Thalidomide exerts its effects by modulating the immune system, including inhibition of tumor necrosis factor-alpha (TNF-α) and altering cytokine production, while prednisolone suppresses immune-mediated inflammation by binding to the glucocorticoid receptor and regulating gene expression involved in immune responses[4].
