Theophylline + budesonide is a combination therapy used primarily for the treatment of moderate asthma and other chronic lung diseases. This combination pairs a xanthine derivative (theophylline) with an inhaled corticosteroid (budesonide). The combination has been shown to be as effective as high-dose budesonide alone, with theophylline potentially helping overcome corticosteroid resistance. Theophylline works by relaxing bronchial smooth muscle through phosphodiesterase inhibition and adenosine receptor blockade, while budesonide reduces airway inflammation. Studies have demonstrated that low-dose budesonide with theophylline can produce similar benefits to high-dose budesonide, even at theophylline concentrations below the recommended therapeutic range, potentially offering a more cost-effective treatment option with reduced systemic effects on cortisol levels.
