Overview

Therapeutic tumor-infiltrating lymphocytes (TILs) developed at St. Luke's Medical Center represent an autologous adoptive cell therapy investigated for the treatment of advanced melanoma. The therapeutic process involves the surgical resection of a patient's tumor to isolate naturally occurring T cells that have already infiltrated the malignancy. These cells are then expanded to large populations ex vivo using high-dose interleukin-2 (IL-2). Once expanded, the TILs are re-infused into the patient, typically following a lymphodepleting conditioning regimen (such as cyclophosphamide) and accompanied by systemic IL-2 to support cell survival and indomethacin to inhibit immunosuppressive prostaglandins. The infused TILs are intended to recognize and mediate the destruction of melanoma cells through their endogenous T-cell receptors, providing a polyclonal immune response against various tumor-associated antigens.
