Overview

TheraSphere + sorafenib is a **combination therapy** consisting of two distinct modalities: TheraSphere, a device composed of yttrium-90 glass microspheres used for transarterial radioembolization (TARE), and sorafenib, an oral small molecule multikinase inhibitor. TheraSphere delivers targeted internal radiation therapy directly to hepatic tumors via intra-arterial administration, using the radioactive isotope yttrium-90 embedded within glass microspheres to selectively irradiate malignant tissue while sparing much of the normal liver. Sorafenib inhibits tumor cell proliferation and angiogenesis primarily by inhibiting the Raf/MEK/ERK signaling pathway and several growth factor receptors including vascular endothelial growth factor receptors (VEGFR-1, VEGFR-2, VEGFR-3) and platelet-derived growth factor receptor β (PDGFR-β). The combination is being investigated for enhanced efficacy in patients with **unresectable hepatocellular carcinoma (HCC)** where sorafenib is started after TARE to maximize anti-tumor effects and minimize overlapping toxicity. Combination therapy is being investigated in large phase 3 studies such as STOP-HCC, with the goal of improving overall survival compared to sorafenib monotherapy[1][2][5].
