Overview

Thiamine + pyridoxine is an oral combination of two water-soluble B vitamins, vitamin B1 (thiamine) and vitamin B6 (pyridoxine), used primarily as a nutritional supplement and as an adjunct in conditions associated with B-vitamin deficiency, including neuropathic symptoms. Thiamine is phosphorylated intracellularly to thiamine pyrophosphate, a cofactor for enzymes in carbohydrate metabolism and neuronal energy production, while pyridoxine is converted to pyridoxal 5′-phosphate, a cofactor for numerous enzymes involved in amino acid metabolism, neurotransmitter synthesis (including GABA, serotonin, and dopamine), and sphingolipid biosynthesis. At pharmacologic doses, these vitamins may exert additional neurotropic and analgesic-modulating effects, including influences on inflammatory pathways, ion channels, and neurotransmission, making them useful as supportive therapy in neuropathic and musculoskeletal pain when combined with other agents.
