A combination of **thiamine pyrophosphate** (the biologically active form of vitamin B1, also known as thiamine diphosphate) and **metformin** (an oral biguanide antidiabetic agent used primarily for type 2 diabetes). Thiamine pyrophosphate is an essential coenzyme for multiple enzymes in glucose and amino acid metabolism, particularly within the tricarboxylic acid cycle. Metformin primarily acts by decreasing hepatic gluconeogenesis and increasing insulin sensitivity, with additional effects on the gut microbiome and other metabolic pathways. Both agents interact at the level of cellular transporters, as metformin inhibits the thiamine transporter (notably THTR-2), potentially leading to lower cellular thiamine levels. There is experimental and clinical interest in combining thiamine or its active form with metformin, in part to mitigate metformin-associated lactic acidosis (MALA) or address metabolic disturbances in diabetes. No uniquely named, co-formulated pharmaceutical product containing both agents is known, but the combined administration (especially intravenous thiamine with oral/intravenous metformin) is described in treatment and research settings[2][3][4][1].
