thiotepa + trastuzumab is a combination therapy being investigated for the treatment of leptomeningeal metastasis in patients with HER2-positive breast cancer. Thiotepa is a cell cycle-nonspecific alkylating agent of the ethylenimine group that works by cross-linking DNA strands, thereby interfering with DNA replication and cell division. Trastuzumab is a recombinant humanized monoclonal antibody that selectively binds to the extracellular domain of the human epidermal growth factor receptor 2 (HER2), inhibiting downstream proliferative signaling pathways and mediating antibody-dependent cellular cytotoxicity (ADCC). This specific regimen involves the intrathecal administration of both drugs, typically via an Ommaya reservoir, to bypass the blood-brain barrier and deliver therapeutic concentrations directly into the cerebrospinal fluid. The combination is currently being evaluated in a Phase II multicenter clinical trial (NCT05003921) led by The First Affiliated Hospital with Nanjing Medical University.
