THR-317 + ranibizumab is a combination therapy consisting of two biologic agents administered via intravitreal injection for the treatment of diabetic macular edema (DME). THR-317 is a recombinant humanized monoclonal antibody targeting placental growth factor (PlGF), exhibiting both anti-angiogenic and anti-inflammatory properties. Ranibizumab is a monoclonal antibody fragment that inhibits vascular endothelial growth factor A (VEGF-A), the current standard of care for DME. The rationale behind this combination is to simultaneously inhibit VEGF and PlGF pathways, potentially providing additive or synergistic effects in reducing retinal vascular leakage and neovascularization associated with DME. Clinical studies have shown that while the combination was safe and well tolerated, it did not demonstrate significant improvement over ranibizumab monotherapy in the overall population but showed potential benefit in certain subgroups such as patients with poor response to prior anti-VEGF therapy or lower baseline visual acuity[1][4][5][9].
