Overview

Thrombopoietin (TPO) and interleukin-11 (IL-11) are both cytokines used to stimulate platelet production. Thrombopoietin is the primary physiological regulator of megakaryocyte and platelet production, acting on hematopoietic stem cells and megakaryocyte progenitors to promote their proliferation and differentiation into platelets. Interleukin-11 is a stromal cell-derived cytokine that also stimulates megakaryocytopoiesis but acts through a different receptor pathway than TPO. Recombinant forms of both proteins—recombinant human thrombopoietin (rhTPO) and recombinant human interleukin-11 (rhIL-11)—are used clinically to treat chemotherapy-induced thrombocytopenia by increasing platelet counts, reducing the duration of low platelets after chemotherapy for solid tumors such as lung, breast, and ovarian cancers, and decreasing the need for platelet transfusions. Both agents have been approved in China for this indication[3][4][8]. Their mechanisms differ slightly; rhTPO acts earlier in the hematopoiesis process with a longer half-life (~40 hours), while rhIL-11 has direct effects on mature megakaryocytes with a shorter half-life (~7 hours)[3]. The combination refers to concurrent or sequential use of these two agents.
