This is a combination therapy consisting of three agents commonly used in the management of acute coronary syndromes, particularly in patients undergoing percutaneous coronary intervention (PCI) for ST-segment elevation myocardial infarction (STEMI): - **Ticagrelor** is an oral, reversible P2Y12 receptor antagonist that inhibits platelet aggregation by blocking ADP-induced activation of platelets. It reduces the risk of thrombotic cardiovascular events such as myocardial infarction and stroke[3][7]. - **Acetylsalicylic acid** (aspirin) is an irreversible cyclooxygenase inhibitor that prevents platelet aggregation by inhibiting thromboxane A2 production. - **Bivalirudin** is a synthetic direct thrombin inhibitor that binds both to the catalytic site and anion-binding exosite of circulating and clot-bound thrombin, thereby preventing fibrin formation, activation of coagulation factors V, VIII, XIII, and platelet aggregation[6]. This triple therapy aims to provide potent antiplatelet and anticoagulant effects during PCI. Clinical studies have shown that using bivalirudin with ticagrelor and aspirin can reduce bleeding complications compared to heparin-based regimens without increasing major adverse cardiac events[1][2]. The combination is primarily indicated for use during primary PCI in STEMI patients.
