Tilarginine + taxane is an investigational combination therapy being developed by Galera Therapeutics for the treatment of metastatic and locally advanced triple-negative breast cancer (TNBC). The combination consists of tilarginine (L-NMMA), a small molecule pan-nitric oxide synthase (NOS) inhibitor, and a taxane-class chemotherapy agent such as docetaxel or paclitaxel. Tilarginine works by inhibiting NOS enzymes to reduce the production of nitric oxide, which is hypothesized to reverse chemotherapy resistance and modulate the tumor microenvironment in TNBC. Taxanes function by binding to and stabilizing microtubules, thereby inhibiting cell division and inducing apoptosis. This combination is administered intravenously and is currently in Phase 1/2 clinical development.
