Overview

This quadruple-action topical ophthalmic formulation is used for the treatment of elevated intraocular pressure (IOP) in patients with open-angle glaucoma or ocular hypertension. It is typically prescribed when multi-drug therapy is required and aims to improve patient adherence by consolidating four medications into a single bottle. The combination integrates four distinct mechanisms: timolol (a non-selective beta-blocker) and dorzolamide (a carbonic anhydrase inhibitor) reduce aqueous humor production; brimonidine (an alpha-2 adrenergic agonist) reduces production while increasing uveoscleral outflow; and bimatoprost (a prostaglandin analog/prostamide) enhances both trabecular and uveoscleral drainage. By targeting multiple physiological pathways of aqueous humor dynamics, the formulation provides a potent synergistic effect on IOP reduction compared to monotherapy.
