Timolol + LCP (Pulsed Dye Laser) is a combination therapy investigated for the treatment of port-wine stains (PWS), particularly in children. Timolol is a non-selective beta-adrenergic receptor antagonist, typically formulated as a 1% topical gel for this indication. The treatment regimen involves applying the timolol gel adjunctively following pulsed dye laser (LCP/PDL) sessions. The rationale for this combination is that timolol may inhibit the neoangiogenesis (re-vascularization) that occurs after laser treatment, which is a common cause of treatment resistance or relapse. This inhibition is thought to be mediated by the reduction of vascular endothelial growth factor (VEGF) production following laser-induced injury. A Phase 3 prospective multicenter randomized controlled trial (NCT01272609), sponsored by the Centre Hospitalier Universitaire de Nice, evaluated the efficacy of adding timolol gel to PDL but found no significant improvement in clinical outcomes compared to PDL alone.
