Overview

Tiotropium + formoterol is a fixed-dose combination inhaler used for the maintenance treatment of chronic obstructive pulmonary disease (COPD). Tiotropium is a long-acting antimuscarinic bronchodilator (LAMA) that works by antagonizing muscarinic acetylcholine receptors, primarily the M3 subtype, leading to relaxation of airway smooth muscle and prolonged bronchodilation. Formoterol is a long-acting beta2-adrenergic agonist (LABA) that stimulates beta2-adrenoceptors in bronchial smooth muscle, resulting in rapid and sustained bronchodilation. The combination provides complementary mechanisms to improve lung function and reduce symptoms such as shortness of breath and wheezing in COPD patients whose disease is not adequately controlled by monotherapy[5][8]. This combination does not replace rescue inhalers for acute symptoms.
