Overview

**Tiotropium + inhaled corticosteroids** is a combination regimen, not a single fixed-dose inhaler, used mainly in the management of chronic obstructive pulmonary disease (COPD). **Tiotropium** is a long-acting muscarinic antagonist (LAMA) that produces bronchodilation by inhibiting muscarinic M3 receptors in the airway smooth muscle. **Inhaled corticosteroids (ICS)**, such as fluticasone or budesonide, reduce airway inflammation through glucocorticoid receptor agonism, downregulating inflammatory gene expression and cytokine release. This combination addresses both airflow limitation and airway inflammation in COPD. Often, treatment is given as part of a triple therapy regimen alongside a long-acting beta2-agonist (LABA), but "tiotropium + inhaled corticosteroids" specifically refers to concomitant use of any ICS with tiotropium, regardless of LABA use. Evidence from randomized controlled trials demonstrates that this regimen can improve lung function, quality of life, and reduce hospitalizations in COPD patients compared to tiotropium monotherapy, though the benefit for exacerbation reduction is inconsistent.
