This is a four-drug combination regimen consisting of **tiragolumab** (a monoclonal antibody targeting TIGIT, an inhibitory immune checkpoint), **atezolizumab** (a monoclonal antibody against programmed death-ligand 1, PD-L1), **cisplatin** (a platinum-based chemotherapeutic agent), and **pemetrexed** (an antifolate chemotherapeutic). - **Tiragolumab** binds to TIGIT and blocks its interaction with the poliovirus receptor, aiming to enhance anti-tumor immune activity. - **Atezolizumab** blocks PD-L1, preventing immune evasion by tumor cells. - **Cisplatin** causes DNA crosslinking and subsequent apoptosis of rapidly dividing cells. - **Pemetrexed** inhibits folate-dependent enzymes involved in DNA synthesis. This combination is under investigation mainly for metastatic non-small cell lung cancer (NSCLC), including patients with high PD-L1 expression and no EGFR or ALK aberrations, and has shown promising activity in phase 2 trials by increasing response rates and progression-free survival compared to immune checkpoint blockade alone[1][2][3][5][7].
