Drug intelligence / Profile preview
Tirzepatide + vitamin B12 is a compounded, non-FDA-approved injectable combination of tirzepatide (a dual GIP and GLP-1 receptor agonist) and vitamin B12 (typically in the form of cyanocobalamin, methylcobalamin, or hydroxocobalamin). It is prepared by compounding pharmacies, telehealth companies, and medspas primarily for weight management in individuals with obesity or overweight, as well as for type 2 diabetes. The addition of vitamin B12 is intended to mitigate common side effects of tirzepatide, such as fatigue and nausea, and to support cellular energy metabolism during caloric restriction. Eli Lilly, the originator of tirzepatide, has issued public warnings regarding the safety of this compounded combination, noting that chemical reactions between tirzepatide and vitamin B12 can generate impurities of unknown toxicity and safety profiles.
