This investigational combination therapy is being evaluated as a neoadjuvant treatment for patients with muscle-invasive urothelial carcinoma of the bladder, particularly those exposed to antibiotics. The regimen consists of tislelizumab, a humanized IgG4 monoclonal antibody targeting the PD-1 receptor to enhance anti-tumor immune responses; cisplatin, a platinum-based DNA-alkylating chemotherapy; gemcitabine, a nucleoside analog that inhibits DNA synthesis; and vitamin B3 (niacin). The inclusion of vitamin B3 is a novel strategy investigated by Sun Yat-Sen Memorial Hospital to potentially counteract the deleterious effects of antibiotic-induced gut dysbiosis on the efficacy of immune checkpoint inhibitors by modulating the microbiome-immune axis.
