Overview

Tislelizumab + gemcitabine + oxaliplatin (T-GemOx) is a combination therapy regimen consisting of an anti-PD-1 monoclonal antibody (tislelizumab) with two chemotherapy agents (gemcitabine and oxaliplatin). This regimen works through multiple mechanisms: tislelizumab blocks the PD-1/PD-L1 pathway to restore T-cell function, while gemcitabine reduces myeloid-derived suppressor cells and increases tumor cell antigenicity, and oxaliplatin promotes immune cell activity. The combination has shown synergistic effects, with gemcitabine and oxaliplatin potentially enhancing the efficacy of tislelizumab by creating a more favorable tumor microenvironment for immunotherapy. Based on clinical evidence: - **Tislelizumab** - An anti-PD-1 monoclonal antibody engineered to minimize binding to FcγR on macrophages, which helps prevent antibody-dependent phagocytosis (a potential mechanism of T-cell clearance and resistance to anti-PD-1 therapy). - **Gemcitabine** - A chemotherapy agent that reduces myeloid-derived suppressor cells, favors reprogramming of tumor-associated macrophages toward an immunostimulatory phenotype, and can stimulate histocompatibility complex-1 expression on tumor cells to increase their antigenicity. - **Oxaliplatin** - A platinum-based chemotherapy that promotes the activity of neutrophils and macrophages and depletes myeloid-derived suppressor cells.
