This is a combination regimen composed of **tislelizumab** (a humanized IgG4 monoclonal antibody against programmed death receptor-1/PD-1), **platinum-based chemotherapy** (most commonly carboplatin or cisplatin), and **pemetrexed** (an antifolate antineoplastic agent). Tislelizumab prevents PD-1 from binding to its ligands (PD-L1/PD-L2), thereby **restoring T-cell function and enabling immune-mediated tumor cell destruction**. Platinum agents cross-link DNA, interfering with replication and transcription, leading to cell death. Pemetrexed inhibits folate-dependent enzymes involved in nucleotide synthesis (including thymidylate synthase and dihydrofolate reductase), inhibiting tumor cell proliferation. This combination is under investigation and in clinical practice, mainly as **first-line therapy for advanced or metastatic nonsquamous non–small cell lung cancer (NSCLC)**, where it has significantly improved progression-free survival compared to chemotherapy alone[1][3][5].
