This is a combination therapy consisting of tissue plasminogen activator (tPA), a serine protease enzyme that catalyzes the conversion of plasminogen to plasmin (the major enzyme responsible for clot breakdown), and bevacizumab, a humanized monoclonal antibody that inhibits vascular endothelial growth factor A (VEGF-A). The combination has been investigated primarily in ophthalmology for the treatment of subretinal or submacular hemorrhage secondary to neovascular age-related macular degeneration and as an adjunct in surgery for proliferative diabetic retinopathy. tPA facilitates the dissolution and displacement of blood clots under the retina, while bevacizumab reduces neovascularization by inhibiting VEGF-A. This dual approach aims to both clear hemorrhage and suppress further abnormal vessel growth[1][2][4][5].
