Overview

This is a hypothetical or non-standard fixed-dose combination of three active pharmaceutical ingredients for ophthalmic use: - **Tobramycin** is an aminoglycoside antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, providing broad-spectrum antibacterial activity against susceptible ocular pathogens[1][4][6]. - **Dexamethasone** is a potent synthetic glucocorticoid corticosteroid that suppresses inflammation by inhibiting multiple inflammatory cytokines and mediators, reducing swelling, redness, and pain in ocular tissues[1][5][6]. - **Tropicamide** is a short-acting antimuscarinic (anticholinergic) agent used as a mydriatic and cycloplegic; it temporarily paralyzes the sphincter muscle of the iris and ciliary body, causing pupil dilation and loss of accommodation. It acts as an antagonist at muscarinic acetylcholine receptors in the eye. While combinations of tobramycin with dexamethasone (such as Tobradex) are widely marketed for treating steroid-responsive inflammatory ocular conditions with risk or presence of bacterial infection[1][4], there are no standard commercial products combining all three agents (tobramycin + dexamethasone + tropicamide). Such a combination would theoretically provide simultaneous antimicrobial action, anti-inflammatory effects, and facilitate diagnostic or surgical procedures requiring mydriasis.
