Overview

This refers to the aerosolized administration of tobramycin and vancomycin as adjunctive therapies for ventilator-associated pneumonia (VAP), as investigated by Wright State University in the clinical trial NCT02478710. Tobramycin is an aminoglycoside antibiotic that exerts its bactericidal effect by binding to the 30S ribosomal subunit, thereby inhibiting bacterial protein synthesis. Vancomycin is a glycopeptide antibiotic that inhibits bacterial cell wall synthesis by binding to the D-alanyl-D-alanine portion of cell wall precursors. In the context of this pilot study, these agents were delivered via nebulization to achieve high local concentrations in the pulmonary system of mechanically ventilated patients, aiming to enhance efficacy against multi-drug resistant pathogens while reducing the risk of systemic toxicities such as nephrotoxicity and ototoxicity.
